LPN Care Supervisor *Exciting New Opportunity
The primary purpose of the LPN Care Supervisor is to provide direction and support to the resident care staff on your shift, along with providing and assisting in the overall care and health services for residents living in Laurelwood Assisted Living, Laurel Way Memory Care, and to those residents with Special Needs Assisted Living by performing nursing functions that identify changes in residents’ health status and making recommendations for necessary actions within the parameters of the DOH regulations.
Performance Outcomes:
- Serves as a clinical resource to staff, residents and families.
- Oversee the evening Resident Care team, ensuring the ALR and SNALR program runs accordingly.
- Assists with medication management and treatments per physician orders.
- Assists with completion of monthly MAR checks and communicating needed changes with the pharmacy.
- Obtains, transcribes and clarifies physician orders. Communicates changes to the team.
- Evaluates resident status and reports any concerns to physicians, families, and team members.
- Assists with training our Resident Services team per policy and procedures and evaluates them as needed.
- Assist with evaluating resident’s ability to self- manage medications by completing monthly audits.
- Reports any med error and/or discrepancies to the RN or Program Director immediately. Conducts monthly audits of the narcotic books and medication storage and MAR.
- Receives referrals and assists in compiling all move-in paperwork- 3122, functional assessments, residency agreements, ISP, etc.
- Helps to ensure all resident ISP, case management notes, and 3122 are current and properly completed.
- Documents in the resident record to reflect nursing interventions and case management activities.
- Assist with resident care.
- Help to coach and mentor staff and serves as a role model for all staff. Assist with on-boarding new members.
- Helps to review current nursing policy and procedures and makes recommendations for new P&P to ensure effective care coordination for our residents.
- Complies with DOH regulations and works towards keeping the team survey ready at all times.
- Perform other duties as requested by the Director, RN or other designee.
Assignments:
*Assigns shift responsibilities to Resident Care Aides and ensure that all assignments for the shift are completed accurately and thoroughly.
*Ensures team coordination, cooperation and teamwork between Assisted Living and Memory Care.
*Ensures adequate staffing for the shift, accepts call-offs and arranges for shift coverage as necessary.
Medication Administration:
*Assists with the administration of medications and treatments as per MD orders and documents assistance in accordance with HAP policies and procedures.
*Assists with the inventory, ordering and proper storage of medications.
*Oversee compliance with Per Diem staff on scheduled shift ensuring all RCA’s follow HAP policies and procedures.
Resident Care:
*Assists residents with personal care and activities of daily living (ADL’s) as per Individual Service Plan.
*Assists in observing overall health status of residents and informing the Director or designee of issues.
*Safeguards medical records of residents, maintains confidentiality and upholds resident rights.
*Assists with recreational activities as needed.
*Assists residents and staff during meal service.
*Answers resident emergency call system promptly, evaluates the situation and resolves as needed, including contacting emergency personnel and/or physicians and families, and arranging for transportation for treatment as necessary.
*Listens and responds to resident concerns and reports issues to the proper discipline.
Housekeeping:
* Performs light housekeeping in apartments and common areas as needed.
* Assists with linen changes and personal laundry weekly and as needed per care schedule.
Documentation:
* Assists the Director of Assisted Living, and RN Case Manager in planning, developing, organizing, implementing, and evaluating resident care services, in accordance with the current rules, regulations and guidelines that govern the Community.
*Completes all necessary documentation including resident care logs, chartable events, shift reports, incident reports and communication logs.
* Ensures the accurate transcription and implementation of all physicians’ orders on their shift.
*Communicates with families, physicians and other health care providers regarding the needs and concerns of residents Performs other duties as assigned by Director of Assisted Living and/or their designee.
Qualifications
Education and Experience: Must possess a high school diploma or GED equivalent, and a valid NYS LPN license and three-five years of prior work experience in an Assisted Living and Memory Care community.
Full time, Varied 7pm-7am shifts, with rotating weekends. *Generous shift differential offered!

Pay range: $25.00hr-$37.00hr.
