Job Summary:
The primary purpose of your position is to provide Social Work services for our residents and their families or significant others.
Develops and maintains therapeutic relationships with residents and their families or significant others.
· Provides to residents and families or significant others; adjustment and bereavement counseling, crisis intervention, problem solving, and concrete services.
· Integrates The Highlands Living Center and Social Work department policies and procedures into daily clinical practices and operations and provides education to residents and families regarding such.
· Advocates for the best quality of life for residents on caseload.
· Acts as a liaison between family/significant others and The Highlands Living Center.
· Conducts and documents psychosocial assessment and interviews involving residents, family or significant other within 7 days of admission.
· Functions as a member of the interdisciplinary care team and participates in resident assessments. Maintain working knowledge of interdisciplinary programs- long term, rehabilitation, and hospice care.
· Adheres to appropriate and timely documentation standards including, psychosocial assessments, MDS 3.0 completion, and associated documentation, adjustment notes, progress note, and discharge note.
· Supports staff around difficult or problematic resident-related issues.
· Coordinated and completes discharge planning of resident in concert with the IDCT.
Qualifications
Education and Experience:
BSW required, MSW preferred from accredited college/university.
1-2 years in Long-term care setting preferred.
Pay Range
$23.00-$34.50/hr
